Business Associate Agreement (BAA)

Effective upon subscription

Howill Health Solutions, LLC (“PI Autopilot”) agrees to the following with each subscriber (“Covered Entity”):

Obligations

• Use and disclose PHI only as permitted by the Covered Entity or required by law
• Use appropriate safeguards to prevent unauthorized use/disclosure
• Report any breach within 48 hours
• Ensure subprocessors sign equivalent BAAs
• Make PHI available for access/amendment requests
• Return or destroy PHI upon termination (where feasible)

Term

This BAA remains in effect for the duration of your PI Autopilot subscription and survives termination for PHI still retained.
